360 Liposuction (Lipo 360): Areas, Planning and Recovery

Lipo 360, also called 360 liposuction, treats the whole circumference of the trunk in one operation, usually the abdomen, waist, flanks and lower back. Fat around the middle forms a continuous belt, so treating every side together shapes the waist from all angles instead of slimming only the front. It suits people with a stable weight, pinchable fat around the waist and skin that can adapt to the new contour.

Why treat the whole circumference
You mostly see yourself from the front, but other people see you from every side, and clothes sit on the waist all the way round. If only the abdomen is treated, the flanks and lower back can look fuller by comparison, and the outline from the side or from behind may barely change. A waistband then still presses into the same roll at the back.
Treating the circumference creates one smooth transition between the abdomen, the flank and the back. In women, the aim is usually a clearer curve between the waist and the hips while keeping the natural hip line. In men, it is more often a straighter flank line without the roll above the belt, and a flatter abdomen.
Which areas a 360 plan includes
A standard 360 plan covers the upper and lower abdomen, the waist, the flanks and the lower back. Depending on where you carry fat, it can be extended to the middle or upper back, the rolls along the bra line or the hips. Not everyone needs every area, and how far the plan extends is decided for each person.
Arms, thighs and the chin are not part of a 360 plan. They can sometimes be added in the same operation, but each additional area lengthens it, so this is weighed against safety rather than added by default.
How a 360 operation is planned
Planning starts with clear front, side and back photos. On the day of surgery, the marking is done with you standing, because the waist, the hip curve and the dimples of the lower back can only be judged properly upright. The marking also shows where fat should stay: some fullness over the hips keeps a feminine shape, and the area just above the buttocks is shaped with care, especially if fat transfer is planned.
During the operation, you are turned so that the back can be treated as carefully as the front. Turning under anaesthesia is done slowly and safely, and it adds time, so a 360 operation usually sits towards the longer end of the 1–4 hours that liposuction takes. The small incisions are usually placed where they are easy to hide, such as in or near the belly button, low in the bikini line and on the lower back or the sides.
If your body-fat percentage is low, high-definition liposuction can be part of the 360 plan, bringing out the muscle contours of the abdomen, flanks and back for a more athletic look.
Lipo 360 with fat transfer
The fat collected during a 360 operation does not have to be discarded: it can be purified and moved to the buttocks, the breasts or the face under the same anaesthetic. A frequent combination is with the buttocks, because a slimmer waist and fuller buttocks enhance each other. This is a Brazilian butt lift, and it brings its own rules, including placing fat only in the layer under the skin and avoiding sitting or lying directly on the buttocks for the first 2 weeks.
When Lipo 360 is not the right tool
Liposuction only removes fat that lies under the skin. If the lower abdomen has an overhang of loose skin, or the abdominal muscles have separated after pregnancy, a tummy tuck, often combined with liposuction of the flanks, is the better choice. If the belly is tight and rounded because most of its fat sits inside the abdomen, liposuction will not flatten it. And if your weight is still changing, the plan should wait until it has settled.
Recovery when the front and back both heal
Recovery follows the general timeline of liposuction, but treating all sides brings a few differences:
Garment: it covers the whole trunk and is worn for about 4–6 weeks. Help with putting it on is useful in the first days.
Sleeping: with both front and back treated, no position is entirely pressure-free at first. Lying on your back with pillows under your knees, or on your side, is usually easiest.
Swelling: it tends to sink with gravity, so the lower abdomen and pubic area may look more swollen than the upper abdomen for a while.
Moving: turning in bed and getting up are sore for the first 2–3 days, and gentle walking starts early.
Sitting: after Lipo 360 alone, sitting is fine from the start as comfort allows; the BBL sitting rules apply only when fat has been transferred to the buttocks.
Work and travel: desk work is usually possible again within 1–2 weeks, and the flight home follows the wound checks, for most people between day 5 and day 7.
By about 3 months the new waistline is easy to see, and it keeps refining until 3–6 months. Frequently asked questions
Is Lipo 360 the same as a Brazilian butt lift?
No. Lipo 360 removes fat around the trunk to shape the waist, while a Brazilian butt lift adds fat to the buttocks. The two are often combined, because the fat removed from the waist can be purified and transferred. When they are combined, the precautions of a BBL, such as avoiding pressure on the buttocks, apply as well.
Can men have Lipo 360?
Yes. Many men carry fat on the flanks and lower back as well as on the abdomen, so circumferential treatment suits them well. The aim is usually a straighter flank line and a flatter abdomen rather than a curved waist. Men with a low body-fat percentage may also be candidates for high-definition liposuction, which brings out the muscle lines.
Does Lipo 360 include the upper back and bra rolls?
Not automatically. A standard 360 plan covers the abdomen, waist, flanks and lower back. The middle or upper back and the rolls along the bra line can be added when there is fat to treat and the operation stays within safe limits. Your written plan should list every area that is included.
